How to Get There

Subway: Take the 2, 4, or 5 train to 149th Street/Grand Concourse Station. Head west on East 149th Street toward Grand Concourse. Walk north on Grand Concourse two blocks, to East 151st Street and turn left. Walk two blocks to Walton Avenue. The PATH office is located at the corner on your right.

  1. Step 1: Gather Mandatory Identification – You must bring identification for every single member of your household. Pack these documents before you go:
    • Photo ID & Proof of Age: Welfare ID, green card, driver’s license, passport/visa, or employee ID.
    • Vital Records: Birth certificates and Social Security cards for everyone.
    • Health Coverage: Medicaid cards.
    • Public Assistance: Public Assistance system identity cards.
    • Proof of Income: Your most recent pay stub if you are currently working.

    Note: If you speak limited or no English, interpreter assistance is available on-site for free.

  2. Step 2: Go to the PATH Intake Center – Travel to the PATH center located at 151 East 151st Street, Bronx, NY (Phone: 718-503-6400).
    • Timing: The facility is open 24 hours a day, 7 days a week, including holidays. However, try to arrive between 9:00 AM and 5:00 PM, as applications are actively processed during these business hours.
    • By Subway: Take the 2, 4, or 5 train to the 149th Street/Grand Concourse Station. Walk north on Grand Concourse for two blocks, turn left onto East 151st Street, and walk two blocks to Walton Avenue. The building will be on your right.
  3. Step 3: Complete the Intake Interviews – Once inside, you will go through a multi-step screening process to ensure you have no alternative housing options:
    • First Interview: Social Services and Human Resources (HRA) staff will review your situation to see if emergency resources (like anti-eviction legal help or family mediation) can solve your housing crisis immediately without entering a shelter.
    • Second Interview: If those programs do not fit your situation, a Department of Homeless Services (DHS) worker will interview you about your housing history.
  4. Step 4: Receive Temporary Placement & Bring Children – DHS will investigate your housing history to make an official eligibility decision, which takes up to 10 days.
    • Initial Placement: You may be assigned to a temporary shelter immediately for up to 10 days while this investigation occurs.
    • Bringing Your Children: Children ages 0-17 do not need to sit with you during the long application process at the PATH center, but staff must view them remotely. You must bring your children to your assigned shelter by curfew the day after you receive your placement (e.g., if assigned on Monday, children must be at the shelter by curfew on Tuesday).
    • Appeals: If you are found ineligible and disagree with the decision, you have the right to request an immediate legal conference at PATH, or request a New York State Fair Hearing within 60 days.

    • Asignación inicial: Es posible que se le asigne un albergue temporal de inmediato por hasta 10 días mientras se realiza la investigación.
